Below you will find two main areas of application for our services.  If you are new to Rainbows you will need to visit the area for New Families.  If you are a family returning to services or currently receiving services visit our Annual Renewal area.  Each section has information on Camp Woodchuck. NEW FAMILIES

In- Home Care Services

Rainbows United offers Supportive Home Care services through the Family Support Services department. These services are provided in the client’s home outside of school hours, during evenings or on weekends. Trained staff is able to assist clients with daily routines like bathing, meals, or meeting a child at the bus.  To apply for in-home services you will need to complete the following steps:

  1. Contact the Sedgwick County Developmental Disability Organization (SCDDO), if you have not previously, to be determined eligibility for services.  They can be reached at: (316) 660-7630. 
  2. Please print and complete forms in the Initial Application.

Additional Information needed for In-Home services:

  • Provide proof of income
  • Plan of Care or Family Support Funding Documentation (this may be obtained from targeted case manager)
  • Person Centered Support Plan (may be obtained from targeted case manager.  If you do not have a Case Manager you may discuss this with Family Support Services staff.)
  • Behavior Plans or IEPs (may be obtained from schools, psychiatric facilities, or specialists.)

 

Center -Based Care Application Process

Center-based care is offered to families after school with our PM Latchkey program, through All Day Latchkeys when school is not in session and on weekends at our Weekend Centers. While in our center-based care, clients are engaged in crafts and activities with peers and staff in a fun and safe recreational setting.  Families interested in receiving Center-Based Care need to complete the following:

  1. Contact the Sedgwick County Developmental Disability Organization (SCDDO), if you have not previously, to be determined eligibility for services.  They can be reached at: (316) 660-7630.
  2. Print and complete forms in the Initial Application.
  3. Print and complete forms in the Center-Based Application.

Camp Woodchuck Application Process

(Camp Woodchuck is from May 29, 2014-August 1, 2014)

Camp Woodchuck is a 10-week recreational camp for individuals with intellectual and/or developmental disabilities who are 6-21 years of age. Campers will participate in fun activities, go on field trips, be part of Talent and Fashion Shows, and have visits from special guests. Campers also have the opportunity to be part of the Voices of the Rainbow choir that performs at area businesses in the community.  Deadline for applications is April 28th, 2014. After this date, completed applications will be accepted, however late applications may receive a start date after June 2nd and hours scheduled will be dependent upon staffing. All applications are accepted on a first come first serve basis.  Families interested in having their child attend Camp Woodchuck need to complete the following:

  • Most recent Behavior Plans or IEPs (may be obtained from schools, psychiatric facilities, or specialists.)

Annual Center- Based Care Application Process

If families would like to receive any Center-Based Care, they will need to provide the following information annually:

  1. Print and complete forms in the Annual Application.
  2. Print and complete forms in the Center-Based Application.
